Our membership model is built around you — your health goals, your history, and your peace of mind. As a Direct Primary Care (DPC) practice, we remove the barriers of traditional insurance and focus on what matters most: providing accessible, personalized, high-quality care.
Services Included in Your Membership
- Annual physical/well child exams
- Acute/sick visits
- Wholesale labs and medications
- Uncomplicated laceration repair
- Incision & draining of abscess
- Trigger point and joint injections (lidocaine and/or steroid)
- Uncomplicated mole, skin tag, and wart removal
- Pap smears
- DOT physicals
Members receive a discount on in-house labs and pharmaceuticals.

Membership Fees
We believe healthcare should be simple and affordable.
For Members
- $100/month (6-month minimum contract)
- $1,100/year (if paid in full at enrollment)
For Non-Members
- $300+ for walk-in/acute visits
- $400+ for laceration repair
- $125 for DOT physicals
Membership Policies
Monthly Billing
Membership fees are collected on the 15th of each month. Memberships will be terminated after three missed or late payments.
Auto-Pay Enrollment
All members must keep a bank account on file and enroll in auto-pay. Due to our small staff, we’re unable to bill an invoice to each patient.
After your initial 6 months, there are no long-term contracts in place. You may cancel anytime, and services will continue through the end of your current billing cycle.
Please note, there are re-enrollment fees of $150 should you choose to cancel or allow your membership to lapse. You will also be added to the bottom of our waiting list, if we have one at that time.
6-Month Minimum Commitment
Upon joining, all new members agree to a 6-month minimum commitment.
Please note that these policies are in place to maintain the extremely high quality DPC services and standards we promise to all of our patients. DPC is about long-lasting, personal relationships with your provider, who knows YOU inside and out. We can’t provide exceptional and personalized medical care if our practice is treated as a revolving door.
We want to help our patients as much as possible. If any financial or other hardships arise, please speak with your provider and they’ll be happy to help in any way they can.
Re-Instatement Fee
A reinstatement fee of $150 applies to returning patients who canceled after their initial membership period.
Appointment Cancellations
Please provide 24 hours notice to cancel or reschedule your appointments. After two missed appointments without notice, a fee will apply:
- A $50 charge for regular appointments
- A $75 charge for appointments that include blood work
Insurance
We do not accept insurance — this helps keep your care affordable and transparent. However, we recommend maintaining a low-cost, high-deductible plan for emergencies, hospitalizations, or surgeries.
Our hope is that these events are few and far between now that you have 24/7 access directly to your provider. But the good news is: we can manage 80% of medical problems in our office or via virtual care!
Medicare
Yes, Medicare patients are welcome.
A one-time waiver is required stating that neither patient nor provider will bill Medicare for DPC services. Medicare will still cover labs, imaging, medications, and hospitalizations ordered by our providers.
Additional Services
Services not included in your membership — such as labs, pharmaceuticals, wellness services, or medical aesthetics — will be billed to your bank account on file at the time of service.
